GSSM Membership Application

To become a lifetime member of Gurdwara Singh Sabha Milton (GSSM), please complete the form below. An initiation fee of $500 is due upon signup. Going forward, membership will need to be renewed as of January 1st of the following year, at a cost of $50 per month to maintain your membership in good standing. After completing 10 years of $50 monthly donations, your membership will become permanent at no cost.
